The SMART-AMI initiative will focus on developing an integrated delivery of optimal medicalcare for patients with ST-Elevation Myocardial Infarction in LHIN IV (Hamilton-Niagara- Haldimand-Brant). This will involve collaboration with Hamilton Health Sciences, regional hospitals, Emergency Medical Service, the LHIN 4 Cardiac Working Group and the LHIN IV Emergency Services Steering Committee Through improved communication, data collection, and rapid feedback loop of key indicators, our goal is to enhance the uptake of evidence-based best practices in managing patients with STEMI in our region, and to provide a “toolbox” for similar regional approaches across the Province.
